You will review incoming vitals and symptom data for patients with hypertension, heart failure, and diabetes, working alongside Cadence nurses and nurse practitioners to act on risk between visits. This role sits inside Cadence's Clinical Intelligence workflows, where alerts are surfaced continuously and licensed clinicians decide what happens next.</p> <p class="font-claude-response-body break-words whitespace-normal" data-sourcepos="13:1-13:532;1233-1764"><strong>This is a part-time, remote position.
