This is an opportunity to build meaningful relationships with patients while improving outcomes for medically complex and vulnerable senior populations.</p> <p><strong>What You'll Do</strong></p> <ul> <li>Conduct in-home and telehealth visits for Medicare Advantage members.</li> <li>Perform comprehensive assessments, medication reviews, and chronic disease management.</li> <li>Develop and implement individualized care plans that address both clinical and social determinants of health.</li> <li>Identify barriers to care, including fall risks, food insecurity, caregiver challenges, and gaps in treatment adherence.</li> <li>Collaborate with physicians, care coordinators, social workers, and other interdisciplinary team members to deliver holistic, member-centered care.</li> <li>Leverage Clover Assistant and EMR technology to support proactive care management and close gaps in care.</li> <li>Monitor health status and intervene early to help prevent avoidable emergency department visits and hospitalizations.</li> <li>Educate members and caregivers on disease management, preventive care, and treatment plans.</li> </ul> <p><strong>Qualifications</strong></p> <ul> <li>Active Nurse Practitioner (NP) license in New Jersey or ability to obtain licensure.</li> <li>National board certification as a Nurse Practitioner.</li> <li>Active DEA and New Jersey CDS registration, or ability to obtain.</li> <li>Experience caring for adult or geriatric populations in primary care, complex care, chronic care management, transitional care, home-based care, or value-based care settings.</li> <li>Strong clinical assessment and chronic disease management skills.</li> <li>Passion for serving vulnerable senior populations.</li> <li>Comfortable utilizing technology and telehealth platforms to enhance patient care.</li> <li>Excellent communication and collaboration skills.</li> </ul> <p><strong>Preferred Qualifications</strong></p> <ul> <li>Home care, home health, home-based primary care, palliative care, geriatrics, or population health experience.</li> <li>Experience caring for Medicare Advantage members.</li> <li>Prior experience in value-based care models.</li> <li>Experience managing medically complex and high-risk patient populations.</li> <li>Bilingual skills are a plus.</li> </ul> <p><strong>Why Clover Health?</strong></p> <p>At Clover, we are transforming healthcare by meeting members where they are.
