She requires 24/7 support, including personal care, companionship, and help with daily living activities. We understand that our employees are our greatest asset</p> <p>Centria Healthcare is looking to hire caring, professional, and dedicated Certified Nursing Assistants and Home Health Aides to join our team.  </p> <p data-start="967" data-end="990"><strong data-start="967" data-end="988">Responsibilities:</strong></p> <ul data-start="991" data-end="1453"> <li data-start="991" data-end="1051">Assist with <strong data-start="1005" data-end="1031">mobility and transfers</strong> to prevent falls.</li> <li data-start="1052" data-end="1132">Provide <strong data-start="1062" data-end="1085">basic personal care</strong>, including hygiene, toileting, and dressing.</li> <li data-start="1133" data-end="1186">Monitor and assist with oxygen therapy as needed.</li> <li data-start="1187" data-end="1235">Prepare light snacks or drinks if requested.</li> <li data-start="1236" data-end="1301">Offer <strong data-start="1244" data-end="1277">companionship and reassurance</strong> throughout the shift.</li> <li data-start="1302" data-end="1362">Assist in positioning and turning to prevent discomfort.</li> <li data-start="1363" data-end="1412">Document and report any changes in condition.</li> <li data-start="1413" data-end="1453">Respond to any emergencies promptly.</li> <li data-start="1413" data-end="1453">Perform duties outlined in patient's plan of care</li> <li data-start="1413" data-end="1453">Monitor patient condition by observing physical and mental condition, intake and output, and exercise</li> <li data-start="1413" data-end="1453">Observe the client’s physical, mental, and emotional conditions/ document and record changes in the patient condition</li> <li data-start="1413" data-end="1453">Maintain a safe, secure, and healthy patient environment</li> <li data-start="1413" data-end="1453">Assist with daily living activities, such as dressing or taking a bath</li> <li data-start="1413" data-end="1453">Adhere to professional standards, facility care policies and procedures, federal, state, and local requirements</li> </ul> <p data-start="1455" data-end="1476"><strong data-start="1455" data-end="1474">Qualifications:</strong></p> <ul data-start="1477" data-end="1915"> <li data-start="1477" data-end="1543"><strong data-start="1479" data-end="1517">CNA, HHA, or equivalent experience</strong> in home care preferred.</li> <li data-start="1544" data-end="1656">Experience working with <strong data-start="1570" data-end="1633">fall-risk patients and mobility assistance</strong> is highly desirable.</li> <li data-start="1657" data-end="1736">Ability to assist with <strong data-start="1682" data-end="1699">personal care</strong> and provide compassionate support.</li> <li data-start="1737" data-end="1808">Must be <strong data-start="1747" data-end="1783">reliable, patient, and attentive</strong> to the client’s needs.</li> <li data-start="1809" data-end="1858">Comfortable working in a private home.</li> <li data-start="1859" data-end="1915">Ability to work independently and follow care plans.</li> <li data-start="1859" data-end="1915">Excellent oral and written communication skills</li> <li data-start="1859" data-end="1915">Ability to follow verbal and w
