This position does not include an on-call requirement.</p> <h2 class="wnfdntn wnfdntm _1ibi0s35e _1ibi0s36a">What You’ll Do</h2> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">As Clinical Manager, you will provide clinical oversight and operational support for a diverse home care caseload, including both pediatric and adult clients. </p> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Responsibilities include:</p> <ul class="wnfdntj"> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Oversee a patient caseload with a range of care needs, from lower-acuity home health aide services to high-acuity care for clients who are tracheostomy- and ventilator-dependent.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Conduct regular client visits and assessments throughout the service area.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Support the delivery of safe, effective, and compassionate care in clients’ homes.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Provide clinical guidance, coaching, and support to field RNs, LPNs, and home health aides.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Collaborate with clients, families, caregivers, physicians, referral partners, and office leadership to promote continuity of care.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Monitor client needs and respond promptly to changes in condition or service requirements.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Participate in care planning, case management, documentation review, and quality improvement activities.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Help recruit, onboard, mentor, and retain field clinicians and caregivers.</p> </li> <li class="wnfdnte _1ibi0s35c"> <p class="wnfdnt10 _1ibi0s3fd _1ibi0s3ce _1ibi0s3eg">Partner with the Director of the office and the broader team to strengthen workflows, communication, and client and employee experiences.</p&g
