To learn more, visit www.habitathealth.com.</span></p></div><p><strong><span data-contrast="none">Role Scope</span></strong><span data-contrast="none">: </span></p> <p><span data-contrast="auto">The Occupational Therapist provides clinical occupational therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants, developing individualized treatment plans, conducting home visits as needed, and instructing participants, caregivers, and staff on maximizing safety and functional independence in Activities of Daily Living (ADLs), exercise programs, and DME use. As a core member of the Interdisciplinary Team (IDT), this role contributes clinical assessments and care coordination in a collaborative spirit that informs whole-person care planning, while maintaining documentation that meets all regulatory requirements.
