You will deliver high-quality, family centered care in the home and community while flexing into virtual acute care nursing to support urgent pediatric needs.</span></p> <p><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">In this role you will:</span></p> <ul> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Travel to members’ homes and community settings to deliver episodic, longitudinal, and urgent pediatric care.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Build trusted, compassionate relationships with children and families, ensuring coordinated, culturally responsive care.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Collaborate within an integrated care team including physicians, APPs, nurses, pharmacists, dietitians, navigators, social workers, and therapists.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Conduct and document in-home assessments; perform procedures such as ear irrigation, point-of-care testing, IV placement and lab draws, wound care, and medication administration.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Provide patient and family education, follow-up care, and care coordination using the Imagine Pediatrics platform.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Flex into the virtual acute care RN role to conduct virtual assessments and manage urgent pediatric needs using established protocols.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Document care in the EMR (Athena) and communicate with families through Imagine Pediatrics’ proprietary application.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Utilize virtual platforms and digital tools confidently to support care delivery.</span></li> <li style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;">Perform other duties as assigned.</span></li> </ul> <p><span style="font-family: verdana, geneva, sans-serif; font-size: 12pt;"><strong>What You Bring & How
